Please be advised that our courteous front office staff will request your insurance card and photo identification at the time of check-in. Your insurance card will be copied for billing purposes.
Payment of insurance copay, deductibles, and non-covered services must be made at the time of service. Payment can be made by checks, cash or credit card. Primary and secondary insurance provided at the time of service will be billed as a courtesy on the behalf of the patient. All balances remaining after insurance payment will be billed to the patient.
