CORRECTIONS:
01:15 – When reading the monograph, I should have stated that the medication can be given by a student nurse.

07:42 and 07:56 – I should have stated that my blunt fill will be going into the sharps

08:00 – I should have gently rotated the syringe after putting the medication in to make sure that medication is dispersed throughout the syringe.

08:19 – Just to clarify, my patients name is Eally Errol NOT Eally Egon.

08:47 – When I was listing the supplies that I would be bringing to the patients bedside, I should have stated that swabs would be included.

10:30 – This is the time when I should have given my patient education on the medication NOT at 12:05.

11:47 – When setting the CIVA pump down to the plunger, I should have stated the infusion time was at 39 min. therefore the CIVA pump was at the right setting.

12:00 - Instead of stating "to flush" should have said "patient maybe getting IV bolus med through the closest port."

CALCULATIONS for Cloxacillin 1g IV q6h via CIVA:
To reconstitute a 2g vial, 6.8mL of SW is needed -
Final volume of the vial: 8mL
Final concentration of the vial: 250mg/mL.

To pull 1g of medication from the 2g vial -
Volume: 4mL

To further dilute, CIVA states 50mL in 60mL syringe -
Volume of NS: 46mL
Final volume of 60mL syringe with NS and Medication together: 50mL
Final concentration 60mL of Syringe with NS and Medication together: 20mg/mL
Infusion time as stated in CIVA chart: 39 mins.
